- If an Association owns vehicles, we collect individual’s driving information (name, driver license number, birthdate or a photocopy of their driver license) for any drivers;
- If an Association has employees, it may provide us with information regarding its workers compensation policy, such as the number of full time/part time employees, classification of employees (clerical, maintenance, lifeguard, etc.), and total annual estimated payroll for each employee classification;
- Information that you provide by filling in forms on our Website; this includes information provided at the time of registering to use our Website or requesting further services;
- Records and copies of your correspondence (including email addresses), if you contact us; and/or
